When major league baseball owners forced Wilham Eckert out of the commissioner's position in 1968, Kuhn, an attorney who had argued cases on behalf of baseball, was the logical choice as his successor. He was visiting all major league cities in his first year as commissioner when he appeared before Dunkers. He held the position for 15 years before giving way to Peter Ueberroth in 1984. He died in 2007 and was inducted into the Baseball Hall of Fame nine months after his death.

A very successful lawyer, Kuhn represented the National League in a lawsuit against them by the City of Milwaukee when the Braves were moved to Atlanta. He emerged as the surprise choice as commissioner when the two leagues were unable to decide between Chub Feeney and Mike Burke. His reign as Baseball Commissioner lasted from 1969 to 1985.

By 1979 much of the focus of baseball commissioner's Kuhn's administration was on the growing drug use of players. Numerous players were suspended. Kansas City Royals catcher Darrell Porter told the Associated Press that he became paranoid, convinced that Kuhn knew about his drug use, was trying to sneak into his house, and planned to ban him from baseball for life. Porter said he sat up nights in the dark, watching out the front window; waiting for Kuhn to approach, clutching billiard balls and a shotgun.
